I was just thinking about this - they are asking for payment upfront before you have even been granted to enter the UK and therefore able to make use of these services. I wonder what the legal position is if your application is rejected - fair enough on not getting your application fee back, that stands to reason - but surely they have refund the 2.5 years worth of these charges if they are not available to you?

Or have I missed that part of the small print?

The Health surcharge is refunded if your visa is refused.
