Flr m extension questions

Hi guys I was wondering if anyone could answer the following:
is the NHS Surcharge fee still payable when applying for extension? What are the fees?
do we need A2 for extension or can we do B2. Doesn't B2 cover both FLR M & ILR Application?
if was applying as self employed. Do I need to show that I have earned 1550 each month. Or do I just need to show i have earned 9300 over the period of 6 months. (So if my monthly earnings are less one month and more the next month)

Thank you

Re: Flr m extension questions

1. Yes, NHS surcharge payable for extentions:

Cost for a year

You’ll have to pay:
  • £470 per year for a student or Youth Mobility Scheme visa, for example £940 for a 2-year visa
  • £470 per year for visa and immigration applicants who are under the age of 18 at time of application
  • £624 per year for all other visa and immigration applications, for example £3,120 for a 5-year visa
- don't know answers with regard levels of English needed
- Self employed - you need to provide documentation to show your business has made 18,600 GBP in income over the past 12 months. i.e your business has had to be viable for 12 months minimum

My wife used the same English certificate, B1, for FLR and ILR.
