Hi everyone!
Quick inquiry regarding the online spouse application (from US to UK). There’s a question which asks if I have ever gone by, or been known by, any other legal names.
I have a maiden name, which was obviously given to me at birth, yet it asks when the name change took place. I may be thinking too much into this, but I’d like to fill in the form accurately! Is it asking when I changed my maiden name to my married name? Or is it asking when I actually took the maiden surname, which would be my date of birth?
I’ve also changed my surname a few times, due to marriage, divorce, and remarriage, but I don’t know the exact dates of all name changes. Do estimations suffice? And do I need to acknowledge all name changes on the form, including going back to my maiden name between marriages?
I hope this makes sense, and look forward to any input from you. Thanks, in advance!

Your maiden name is not a 'name change'. Subsequent changes would normally be linked to marriage date(s), divorce date(s) - assuming you resumed use of your maiden name), and/or date of legal name change (if any).
It's been a while, but as I remember, I put my maiden name, then 1st marriage, then 2nd marriage dates. After my divorces, I did not resume use of my maiden name. Since you have to provide a copy of the divorce decree, that should give you dates if resuming use of your maiden name. Likewise, I assume you know the date of other marriage(s), or can check public records (marriage licenses or similar).

Thanks for your reply! I did resume use of my maiden name between the 1st and 2nd marriage, so perhaps I’ll just insert my maiden name usage for that time period.
Again, thanks for your input! Much appreciated.
