Visitor stay expired but MEDICAL surgery means have to out stay...what to do?

Visitor stay expired but MEDICAL surgery means have to out stay...what to do?
Hi there,
I came to Canada at the beginning of Jan this year and was due to leave on 23rd March. However, I broke my back, had intense surgery and am now in a back brace until May 1st. Where do I apply to get permission to stay until then? I have documentation from my surgeon stating that I am not allowed to fly until May 1st. Also what are the restrictions of me crossing the USA border and back? I wanted to go to Seattle for the night next week but want to make sure I can come back into Canada.
