I have been trying to confirm what this form is for, can you shine light on it ?

You use it to claim tax back.
When you are on PAYE your tax allowance is worked out per month , and it assumes that you will be resident for a full tax year. If you are not you are due a rebate
I got back about 800 UKP so well worth it

Here is one if you need it. Just fill in and take to local tax office and they will put it in their internal mail. Or you can post it.....
