If you can get a copy of it, then great, otherwise, just provide full
disclosure of the incident, the operation and all that you remember and/or
have any documentation about. A current medical evaluation from your
current physician may also be heplful. If they are not satisfied with it,
they may ask you to undertake additional tests/evaluation - this is likely
to cause delays.

Medical Report/Tests
 
My husband has his medical today. Thirteen years ago he had a head operation due to a football accident. The DMP said he needed a copy of that report to submit to London. My husband doesn't know if he can get a copy of it.

Can't the DMP just write the details of the incidient and operation on the report? Has anyone had Immigration insist on having a copy of this type of report? It doesn't make sense to me as he has been perfectly fine with no complications for 13 years and it is not a threat to the health and welfare of Canadians??

We're worried this will delay the processing of his application.
