We used the templates....the important information is not much. Company letterhead, policy number, exact dates of coverage, name of insuree I think and some sort of official signature from the insurer. And of course the fact that no claims were made during the life of the policy. If you ask for these very specifically from the insurer and explain why you need this exact information they mostly oblige. We had to ask for at least one to be redone, and had to beg for a couple more by phone, but we did get them all in the end. Good luck.
