There are some requirements on the part of the employer though with regards to turnover etc (that I don't think would apply in a normal LMO application), so if it is a (for example) small family run company with not great revenue, they may struggle to get accepted.

No for Ontario you do not have to be working in Canada to be eligible. You don't need to use the TWP option that would be available to you once you receive nomination from them - the idea was that you would receive PR alot faster than if you went the FSW route - therefore not having to wait 12 - 18 months to get out here and start working for the employer.
