Filling passport renewel form

I am trying to complete the form to renew our british passports through the Washington office, but I am a bit confused with some of the questions on the form. Any help would be appreciated.
My son was born in 1983 so we have to fill iin the parents details in section 2.
I have put all our details but it want place and date of issue for our passports. Date is no problem, but what do I put for place? Is it Britain, England? not sure where it was processed precisely.
Section 9 deals with countersigniture and photographs. Where can I find the specifications for the pitures? Its easy in england cos you just pop into a photobooth and get some done.
Confused. Please help as I have to get this form off asap as we are going to Mexico at the end of March and I know I am cutting it fine.

My old one said "Passport Office London" I think. The newish one issued through the washington process just says "FCO." OH's one, issued just before we left the UK, says "UKPA." I only know this because I happen to have them on my desk...

Yes mine says UKPA under Authority so I guess thats what they want.
