Default Exchanging driving licence in Ontario

I have read the wiki page on this but I am still slightly confused, so please forgive me for seeking clarification.

To exchange a UK licence which has been held for over 2 years, do we need to fill out the D888 DVLA form to get a confirmation of our driving record or is this not required?

it wasn't required when i did mine in Ontario, they just wanted my old licence,but that was over 10 years ago.

Hi!

I changed mine 6 months ago in Ontario. I took a print out of my licence record I downloaded from the DVLA which they glanced at but really weren’t interested in.
https://www.gov.uk/view-driving-licence

In addition I need to show:
  • UK licence (just the card – I didn’t bring my paper licence)
  • My Visa
  • Passport
  • Proof of address (we got a letter from the bank as we didn’t have any bills or anything at that stage)
They took my photograph and I paid $85 and that was it.
