CRNBC registration question
 

I am really confused. I have sent my application pack to CRNBC but my status on the site says that I am missing some things

1) application form
2)copy of ID
3)Professional Experience Record
4) Form 11 (statutory declaration)


1)does anyone know if the "application form" is the original one (which I'm sure I did right) or another one?

2) do copies of ID need to be certified?

3) do they ask for this after the initial assessment?

4) I actually know that Form 11 this goes off later!
