Holy god, that's terrible for a kid to have to go through that.

Glad he's been seen to now, but how ludicrous not to be able to access an emergency dentist, even with comprehensive health insurance!?!

LG the nitrous certainly sounds like a good option as hearing your son scream blue murder is not nice and having to pin him down makes me feel awful.

Definitely have a chat with the dentist before you go next time. As my dentist put it "I'll give you whatever drugs you want if they stop you from punching me or biting my hand". After I made one or two wisecracks about him being a scouser and asking that he remembered to put metal into my mouth instead of taking it out, I'm surprised he didn't sedate me with cyanide!
