If you have a spouse you can make a combined application. If not married, you need to have cohabited for at least 12 months.

I thought if you are married she has to be on your application anyway as i have the reverse problem,ie im trying to emigrate but my wife is to ill to come with me but still has to be on my application!

If you are not married but have cohabited for 12 months you are generally considered to be 'married' as far as immigration law is concerned.

If this still confuses => you need a good agent if serious about migrating.

Spouse doesn't *have* to be on your application (although usually this is advisable), but whether included on your application or not, a spouse must pass health/character clearances otherwise you will not get a visa.
