The mystery of part E! (med forms)
We've printed off our medical and xray request forms from an offical email from our CO, only on form 26 (medical form) there is no page 10 or part E and likewise there is no part D on Xray forms (106) Confusing or what as it clearly states on front page of each that we are required to complete these parts.
Any ideas, we have had our Xrays done ignoring missing part D, only am worried this will become a problem at a later date.

Parts D & E were tear off strips at the back where you just re-entered your name and address type stuff. Don't worry about it. DIMIA will catch up with themselves soon

Along with our med request the e-mail from our case officer said not to worry about those parts of the form as the notes that accompany the forms had not been re-written yet.
