hi to all out there,just a short question,we are having our medicals in august and have been reading the website reference medical forms no 26 and 160 for xrays could not find these forms on the website will our dr have them, we are going to one of the drs on the list

It seems like a lifetime ago,but I think I emailed DIMIA for three sets of each form and they arrived in the post about 7 days later.

You can't download them as the Doc has to seal them before sending them on to you/ direct to DIMIA

Hi Evista, the following is taken from the DIMIA web site :-

Medical and Radiological Forms
Medical Form 26 and Radiological Form 160 are not currently available in electronic format. To obtain these forms you should send an e-mail to the firstenquiries address below.

E-mail: [email protected]

The following information should be provided:
• name
• postal address
• the visa type you are applying for
• the number of applicants included in the application
• the age of each applicant included in the application.

The docs and xray people will have them anyway.
Just phone them to check - ours did, so would assume that to be the norm.
