Medical forms, where to get

Medical forms, where to get
Hi all,
I've seen a lot of people sending front loaded Med/police check apps, but where do you get the forms from? (I'm not front loading, just pre-empting)
I've contacted a surgery on the list, and they say I must bring the 'forms' with me.
I haven’t had the med requests yet, although they are due soon (applied March 04) and I want to book/do the meds/x-rays ASAP, while I have time and speed up the process.
If anyone knows which forms, and where I can get these forms from, can you post a reply?

I sent an email to the AHC in London [email protected]
Just said "can you please send me 3 medical forms" and included my name and address and they arrived a few days later.
