I can confirm that NAME was employed/is currently employed by company name of address from/since month/yr to month/yr/ OR present as an exact job title (ie. Apprentice/ trainee hairdresser) and from/since month/yr to month/yr/ OR present (as an exact job title ie. Fully qualified hairdresser)

(if more than one job title then split into each different job title with exact dates below).

He/She worked full time/part time and worked approximately hours per week. His/Her normal working hours were from time am to time pm , day to day. His salary/wage/hour is _____________

The company’s core business is to provide details of what the company does or what kind of company it is. NAME provide details of what the prerson generally did.
