help form 26
Hi everyone
Due to have our medicals really soon - however our panel doctor told us he doesn't have form 26 and we have to bring it with us (we will easily find it on the internet!) after numerous searches the only 26 we can find is for on shore applications. Does anyone know the link for the correct form? Panicking now - already got my husband convinced his medical is going to uncover several unknown about tropical diseases don't need this added stress!!

Copy of the one we used (in the UK) attached below. No problems - visa granted. Don't think there is a distinction between on and offshore??
