You need Form IR85, obtainable from your IR office, send in IR85 before or with P45 and the Revenue will sort refund or payment required.

The form P85 (link as kindly provided) - is this only for employed or can it be used for self-employed. If cannot be used does anyone have any ideas what form is necessary for self-employed?
