Child medical forms???

Child medical forms???
I go for the second part of my medicals in approx 15 hours and my son who isnt going with me has to get his done too. Im using form 26 and even though a few weeks ago i was sure my sons med could go on this form, well, im not too sure now. Is there a form for a childs medical, who isnt migrating. Im in the uk btw. I really would appreciate any help with this.

As far as I know you have to fill one in for everyone.

Yes, but a seperate form, not on yours
