Form DS-157 (Supplemental Nonimmigrant Visa Application) is a supplemental form for Form DS-156.

As of January 18, 2002 all male visa applicants between the ages of 16 and 45 are required to submit Form DS-157 in conjunction with Form DS-156. Each Consulate or Embassy also has the right to request DS-157 from female applicants at their discretion.
