Default UK iPhone 5 what sim in America.

As it is, I'm on O2.
I don't very much care for the roaming charges...

I can get a PayG sim in America now, right? (I couldn't before, years ago when I went as a teen).

Has anyone put a US sim in a UK phone and found it worked with only AT&T or T Mobile and, does the phone have to be unlocked first?

This has been discussed many times. There's a handy search feature near the top right of each page... I suggest you use it. It's the difference between spoon feeding you information and you being responsible for your own research.

I understand. (I'm a card carrying conservative).

Yes, any European SIM-based phone should work on AT&T or T-Mobile as they are legacy GSM networks. The phone will definitely need to be SIM unlocked first.

Why ATT and T Mobile? I suggest using Cricket (owned by ATT so same network) except cheaper and true pay as you go is available - so none of those annoying trailing taxes that suddenly make your bill 5, 10 or 20 more dollars than you expected. I use a $35 Cricket plan and am able to do everything I could on my previous ATT $$$$ extortion plan. That includes Skype, email, snap, text etx and yes I'm able to use back to the UK to.
