Yes, you do. It is your number for life and if there is a place on any forms/petitions for it, please insert that number where appropriate.

After you have successfully done your adjustment of status to permanent resident and have your green card in hand, you then return to the social security office and have the work legend removed. They will send a new card, same number, but without the legend.

Yes, like Rete said, the number is yours for life and you cannot change it even if you wanted to, at least not unless you are accepted into the Federal witness program and given a new identity.
