Default IRS Form 8802

Does anyone know if this form needs to be filled in each year? The Instructions aren't very clear. Currently in the process of submitting US2002, and came across this additional form 8802 that needs to be processed along with US 2002.

I agree - instructions are vague; I had same concern a few years ago when I filed it.

I have another question - but about form US-Individual 2002. Question C2 asks for 'Nationality'. As a naturalized US citizen still holding a UK passport - what determines nationality. Any help here would be appreciated.
