Obtaining Passport

I am a Canadian citizen, pursuing a masters degree in Oklahoma. I have an F1 student visa.
When I entered the country, I presented my I-20 and received a stamped I-96 form. I used
my birth certificate and drivers license as identification, since I was told that I didn't
need a passport.
Now that I am in the US, I feel that I should obtain a passport. How should I go about
obtaining one? Could I just send my social insurance card back to my mother in Canada,
along with a passport photo, have her fill out a passport application for me, and send it
in? Then, when they send my passport to her, she could mail it to me. That seems like a
simple solution.
Still, I'm not sure what to do. Which address which I have to provide on the passport
application form? My old Canadian address, or my current US address?
Any advice would be greatly appreciated.
