It depends on which Embassy and how busy they are, usually several weeks at least.

Highly variable. First of all, it depends on the consulate. Second, it
depends on the individual case. Especially if you are from a predominantly
Muslim country, expect substantial delays. If you are from a poor country
or a visa waiver country, expect a denial - some B-1/B-2 are approved, but
you will need some special reason and/or plenty of evidence that you will
return.
