"Are you OK?"
The question asked to people who appear to not be OK.
Why the f$%^k would you ask that when you see the answer to obviously be no and when your question either at face value or otherwise does nothing to help the other person.
Heck, they even ask this question to people who seem to be in serious health distress (fainting, vomiting, etc_

That's a good point. The next time my daughter trips and falls, I shall now resort to pointing and laughing instead of asking if she's OK

Because it gives the person a choice to say no I need help or yes I'm ok. There are other things to say and that's just one of them.

I don't understand. Your comment and the others, on this one. I don't.

In the case your child falls and is showing no outward signs of distress, the question - I think - is fine. But my comments were when this Q is asked when the person is obviously in distress.

To me, the question "are you OK", puts the pressure on the injured person to say, yes, I'm OK.

I would ask any one of the following, as appropriate
are you hurt
where does it hurt
what happened
can I help
what's wrong
etc
