The water park in Kansas that killed a boy a few years back, the park, a manager, co-owner and others are facing various criminal charges.
Turns out it appears no engineers were involved in the rides dynamic engineering or slide path design, and those who decided to design and build it had no expertise in such a task.

I put this one down to poor training. There's not really another explanation for him not to use less lethal force with an unarmed, and obviously hampered (by the trousers round his ankes) man here.

No need to use any kind of force, just go up and talk to the guy, and get him to a place of safety. (Mental hospital for assessment)
