The biggest "trap" being for those at the bottom of the income scale who live in states that refused the Medicaid expansion.

Yes.. which is why I was confused by Boiler's post which to me makes it sound like a working person can simply opt out as they please.

VAT? Gas taxes? Other excise taxes? Pretty difficult for a resident not to be paying at least something in general taxation.

Or non residents.

Wonder how many resident in the US do not in someway contribute to the NHS, and vice versa for say Medicaid.
