From what I understand of insurance approvals, when you are denied certain treatments etc, you can request to speak with the HIPAA Compliance Officer at the insurance company. From there you find out who was reviewing your file and who made the decision. Often it is unqualified staff - with no medical knowledge, reviewing and denying cases. Using this information, it is entirely possible that you could also request a review of the decision, as an unqualified person would have been making the decision.
