Old thread but there may be others who pass by the forum with the same question. I would only ever use 'Cheap as Chips' in Patong / Phuket. Great company, run / owned by a Thai lady (Cat) and her Australian partner (Brian). Good range of bikes, good prices, rental contracts in English, no bullshit and no scams!
They also own / run 'Phuket Travel and Tours' if you need trips etc, again, no rip offs.

Personally, I would never dream of riding a motorbike in Thailand. It's dangerous enough driving a car here. (Look at the statistics for deaths on the roads here - it's mostly people on motorbikes)
If there's an accident, you're likely to be taken to hospital in the back of a pickup rather than an ambulance, even in Bangkok. That's not the best method of transport for a spinal injury.
Also, if you still want to ride a motorbike, get the IDP and have bikes on your licence. If you don't have a valid licence for Thailand, in the event of an accident, the insurance won't pay out and you'll be personally liable for any damages.

Even the government insurance that you have to pay yearly by law is worth doing but many don't . I fractured the bone going into the shoulder a few years back and it covered nearly all of the bill . It's only 3-400 a year too.
