What document do i fill in at POE on Advanced Parole AP

I am a UKC currently awaiting AOS
I have my EAD and AP combined card.
I need to leave USA for a week next week.
What form do i fill in upon reentry ?
Is it the white I-94 ? If so what do i put on the form for where visa was issued and visa number etc.
Any help greatly appreciated.

I-94 (See the CBP IFM, Section 16.1(b)) and 6059B.
Good question - I forget how my wife filled it out back when.

Thanks for quick reply Jeff.
I am coming into a small Cruise terminal and fear they may not be as familiar with the AP/EAD card as those at major airports.
I cant see how i can answer a lot of the questions on the I-94 though as they ask visa number and where issued etc.

Indeed. See the CBP IFM Section 16.1(c), which seems to assume that a person arriving at a Port-of-Entry will not have advance parole but rather will be applying for parole. But surely there will be some CBP officer there, perhaps at the supervisory level, who will be able to deal with it.
N/A may be the correct answers to the questions, but in the end you'll write what the CBP officer wants you to write.

I'm pretty sure you don't fill out anything other than a customs form.

You might be right, but the IFM instructs CBP officers:
"Once you are satisfied that the person is entitled to parole, endorse the I-94 with the parole stamp, indicate in the appropriate block the basis of parole (e.g. “I-512, adjustment applicantâ€), the date to which paroled (or indefinite), the date of action, port and your stamp number."
I seem to recall that my wife was paroled in for 1 year after she had visited her native country when she was adjusting status.

I get the feeling we are going to face a minor problem upon arrival . It will be just my luck if the "supervisor" is off that day!

I doubt that. From my memory They took mine, stamped it and that was it. It's not an uncommon document and any airport that accepts international flights will have had plenty come though before you.

I have the new EAD / AP combined card. I think that may confuse them particularly as i dont know what form to fill in (although i agree with people on here it will be the white 1-94).
Plus i am coming off a cruise and i think we clear immigration on board, so it will be an issue i think. But hopefully it will all be sorted out.

We came through immigration off a cruise once with AP docs and the immigration officer didn't have a clue. Said he thought he might have to send us back from where we'd come from...seeing as it had been a round trip from Galveston we were a little confused, and asked for the supervisor, who knew exactly what he was doing.

When people "hear horror stories", they usually end up actively seeking out horror stories that support their fears! The reality is that for every horror story you hear, there are thousands of people who have no problems whatsoever.

I agree, just me being paranoid..
Just heard that the sister cruise ship we are on had hundreds of passengers quarantined yesterday with nor virus so i guess reentry will be the least of my problems!!!
