On official forms, no you should be fine. On sworn statements or anything you've typed yourself I would put the information down on things EXACTLY like they did.

There is. I went for my J1 in '08, and took photos I did myself at home with me. They weren't suitable BUT literally round the corner from the windows where they interview you is a photobooth.
