Thank you Szeged - that is exactly the card I have ! There does not appear to be any expiry date on it either.
Does that mean I will have to do a separate application for the address card or will...

Hi Szeged,
It is a laminated card, light brown at the top that turns to green further down and has my name and address on it and a number in the bottom left corner - it also has on both sides...
