Ours is with BoA, but it was opened up via my wife's employer. I assume they do individual accounts too.

The money is taken pre tax every month, then her employer adds some of their own...

As for cost, mine is for two people and subsidized by my wife's employer so can't really draw comparison.

Why would you do this as what you describe a healthy person?
