In the application leaflet it just mentions the following (see Point E no figure): If you want to move to UK, maybe the Surinder Singh Route might be an option but other members in the UK section...

As far as I can see is that your spouse would need to fill out following form: http://www.inis.gov.ie/en/INIS/Form%20EU1.pdf/Files/Form%20EU1.pdf

You should apply as a Qualifying Family...

You can apply for a single journey short-stay C visa which will permit you to enter and reside in the State for up to 3 months.
