Interesting that they have two places in Bahrain even though the whole island is not much bigger than a football pitch

I would have guesses New Delhi, Mumbai,Karachi , Beijing to be above the top 5 on that list.

I think the issue is PM10 - this is particulate matter, normally from diesel engines.
Beijing's air quality is actually a lot better than many people think(!) - while it is shockingly terrible, some of it is as a result of fog (I've been three times, and on two occasions it was beautiful). It has also improved a lot in the last few years.
That said, if the chart was PM2.5 or NOx, Beijing may well feature much higher.

Dammam/Dhahran/Khobar are bad. You can see the haze over the motirway system Emissions for cars. Jubail is also bad - ditto plus gunge from petrochemical plants. Medic there told me that he had never seen such a high incidence of breathing problems anywhere.
