Where do I find my DHS "A" number and AOS DHS number ?
 
Folks, please help with the following (I need the info to fill out
Form 7765: Evaluation of Returning Resident Status for an SB1 visa):

1) Where do I get my DHS "A" number ?

2) Where can I find (a)Date of Adjustment of Status with DHS (b) Place
of Adjustment of Status with DHS ? The date and place of AOS are of
course known to me/stamped on my passport, there is also a scrawled
number here which is not fully legible, is this the DHS asked for ?

3) To refresh my memory, is the H1 a non-immigrant visa ?

Out of curiosity, can I get my visa record (i.e dates of all
entries/exits to/from the US) somewhere ?

on your plastic GC, or on your I-551 stamp in your passport

2) Where can I find (a)Date of Adjustment of Status with DHS (b) Place
of Adjustment of Status with DHS ? The date and place of AOS are of
course known to me/stamped on my passport.

If you remember your place and date of AOS, why all additional questions?

3) To refresh my memory, is the H1 a non-immigrant visa ?

Yes, it's a non-immigrant visa.

Thanks, so the "INS A#" and "DHS A#" are the same ... Apropos my
second question, I wanted to know what "AOS with DHS" referred to.
BTW, what does "DHS" stand for ?

I have appended the relevant portion of the form in question, Item 8d
is my area of concern:

Well INS doesn't exist anymore. It got split up into 3 parts, USCIS, BCBP,
and BICE, all are under the umbrella of DHS...

DHS = Department of Homeland Security
