Default Portuguese Health Service - for Residents

Can someone help help/clarify please?
My brother-in-law has resided in the Obidos area of Portugal for several years, and holds a Portuguese passport.
Last week he went to register with the local NHS provider, only to be told that before any tests/treatment could be done, they required his EHIC so that they could bill the NHS in England.
Any help on this would be appreciated.

Does he have a Cartão de Cidadão and is he known to the authorities as a Portuguese resident?

EHIC card is for residents of other EU countries to get treatment here or in other member state countries.

If he is a registered resident in Portugal he's entitled to the same more or less free healthcare as any Portuguese person.

I was never asked for a EHIC Card nor was my wife. We don't even have those cards but we both have used the services with no problems at all.

The OP's brother in law holds a Portuguese passport so he is a citizen of Portugal so why would he need a EHIC Card???
