MAF and Customs declarations - not sure PSS are advising us correctly!
PSS have requested only a basic form in their own format for "outdoor" items.
I have hard copies of two other forms, different to each other but both asking about cleanliness of outdoor items and both seemingly on official letterhead from NZ customs.
Which ones need to be submitted, to who, and when?

From memory (September this year) PSS required the basic cleanliness declaration form filled out before shipping. We filled out further detailed forms on arrival in NZ that their local agents (NZ Van Lines) had sent us and were to be returned to. No separate forms had to be sent direct to Customs or MPI (aka MAF).
If you're bringing over a hoover, make sure PSS label the box it's contained in! They didn't for ours, so on delivery we had to find and return it to MPI and are still waiting to get it back.

We are just importing our camper and a few boxes, Anglo Pacific sent us all the forms required but we have just been sent new forms by NZ Vanlines as the ones we filled in were outdated and would not be accepted by customs, it's not arrived yet so hoping we get it cleared before Xmas. Our form just asked how we had cleaned items and to add box number it's packed in on the form
