I'm posting this because the question keeps coming up.
My OH has just received his driving licence, having exchanged his Canadian licence. All he had to do was tick the box on the form saying that he passed his test originally in the UK, and his licence was exchanged without the need for him to retake his test.
That was what we expected.

did your OH have to pass the drivers test in Canada to get the Canadian license in the first place?
In the exchange process, did your OH get a 'Full' UK licence that allowed OH to drive standard?
Was there any proof required that OH had passed the UK test previously?

He took a test in Canada when he was a teenager. He took another test in the UK when he first came here 30 years ago. After that he did not have a Canadian licence until he returned in '06
When we went to Canada in '06 he exchanged his UK licence for a Canadian licence. And now he has just exchanged the Canada licence for a UK one.
The question comes up occasionally because people think that if they exchange a Canada licence for a UK one they will only be licensed for automatics. The wording on the DVLA site is a bit unclear. In fact, the form asks where you took your test and if you tick UK it will give you a full licence.

I'm assuming they do some sort of check and don't just hand out a full licence on the applicant's word that he passed the test in the UK, but OH wasn't asked to provide any proof himself.
