Smile Carers of disabled kids in Malta

Hi there,
I am looking to move to Malta with my two daughters but my youngest daughter is severely disabled and needs a lot of care. I am looking for some information on the financial assistance that we can get if we do move to Malta. We vacation regularly on the island and have friends who live there as well but back home in the Uk we feel very isolated as family and friends don't really interact with us any more since my daughter was born.
She is 10 years old now and I would be looking for information such as specialist schools, assistance she could get when she is older etc.
Can anyone point me in the right direction or give me some advice please? We really want to make this dream of living in Malta a reality but need lts of advice first.
