Standart forms to inform CHC London about changes

Hello dear experts!
I just wonder - are there any standart forms for applying with changes after you've sent
the general package. I remember that in this conference quite long time ago it was a
message with attached standart form for address change. I tried to find it but failed. And
actually I need to inform CHC London about my new passport and my new residence permit for
country where I am living right now. If there are no that kind of forms, should I send
this information by fax (and in this case the quality of copies from passport will be
quite bad) or by mail?
