Default Passport request, decision made- biodata?

Hi,

We change to decision made today and got the email with passport request. Email mentions biodata but it doesn't have a request form, so is it needed or is it just included in the standard email?


STEP 2: PHOTOCOPY OF PASSPORT, PICTURES AND BIO DATA

We require A PHOTOCOPY of passport and 2 Immigration size pictures with the specifications listed on the following link: (http://www.cic.gc.ca/english/pdf/kit...998E_tempA.pdf). Pictures are required for you and for each accompanying family member.

DO NOT SEND YOUR ORIGINAL PASSPORT. You are a citizen of a visa exempt country.

The biodata form is right at the very bottom of the email (after the French translation); I almost missed it at first.

So that's it just height and eye colour? no finger prints etc?
