I heard that if somebody stays in UK for 48 months in work permit he can apply for permanent residency in UK. Can anyone tell me whether this 48 months needs to be continuous staying or total stay during any length of time?

The time spent in the UK on a work permit to be eligible to apply for ILR must be 4 years continuous stay...
It cannot be accumulated over different time periods.

I heard that less than 6 months gap (staying outside UK) is usually considered continuous.

Usually periods longer than 3 months are considered to break the continuity of time spent in the UK.
